Incident Report Form for
HAI Global and HAI Canada
This form is to report violations that occurred at HAI events, or by HAI personnel.
Your email will be read promptly, and you should expect a response at least within three business days (and often faster). If you need support more quickly, please contact your regional Producer, the Norcal HAI office, a Facilitator, or HAI Global’s Executive Director. If you need assistance filling out this form, please contact safety-and-consent@hai.org. You may fill this form out for yourself or on behalf of another.
For all regions, this form will be sent to HAI Global’s Safety and Consent office. At their discretion they will contact the regional Producer(s) as needed.
For incidents where Ontario, Canada is chosen as the region, it will be sent to HAI Canada’s Senior Group, which consists of the HAI Canada Board Chair, Executive Director, Ontario Producer, and the HAI Global Facilitator Liaison to HAI Ontario. In addition, the HAI Global Safety and Consent office will be notified.
For incidents in Germany/Europe, you may use the form below or contact our HAI Germany Producer directly via our Contact Form. If you use the form below it will be sent to the HAI Germany/Europe Producer and the HAI Global Safety and Consent office.
Initially, only the Safety and Consent Officer and their Safety and Consent Partner see the report. Their first action will be to contact the person who brought the issue forward and see if they want a conversation. Further action depends on the results of that conversation.
In addition to this form reaching those mentioned, a copy will also come to you at the email address you provide. We recommend that you do not use a work email address, or a shared one unless you are comfortable with others reading all that you write here.
